General Instructions:
Sales Tax Dedications
Appropriation recipients please complete the Cooperative Endeavor agreement
form and the attachments to the Cooperative Endeavor agreement form, both in
Word format.
Please submit the
completed Cooperative Endeavor agreement and attachments via email to: wwaits@treasury.state.la.us
Two originals of each signature page (Cooperative
Endeavor signature page, Attachment E, Attachment E-1, if applicable, W9 Form,
and Board Resolution, if applicable) should be mailed to:
