General Instructions:

Line Item Appropriation recipients please complete the Cooperative Endeavor agreement form and the attachments to the Cooperative Endeavor agreement form, both in Word format. 

For Line Item Appropriations under Schedule 20-945, please submit the completed Cooperative Endeavor agreement and attachments to:  atolliver@treasury.state.la.us .

Two originals of each signature page (Cooperative Endeavor signature page, Attachment E, Attachment E-1, if applicable, W9 Form, and Board Resolution, if applicable) should be mailed to:
